What are Uterine Fibroids?
Uterine fibroids or leiomyomas are the benign (non-cancerous) growth of the uterus that often emerges during childbearing years. Uterine fibroids often cause no symptoms and are not likely to develop into uterine cancer. These fibroids develop from the myometrial wall (muscular tissue wall) of uterus. On Histopathological examination, uterine fibroids appear as small rubbery mass of tissue having different pattern of growth. At times, some uterine fibroids shrink and disappear on their own, while other might need surgical interventions.
What are the signs and symptoms of Uterine Fibroids?
Most of the women having uterine fibroids are asymptomatic and fibroids are usually revealed accidently during a pelvic examination or pre-natal ultrasound. The common sign and symptoms associated with uterine fibroids are:
- Prolonged menstrual bleeding ( more than seven days)
- Heavy menstrual bleeding
- Spotting or bleeding between periods
- Pain or feeling of heaviness in pelvic region
- Persistent pain during intercourse
- Increased urinary frequency
- Difficulty in voiding urine
- Enlarged uterus or abdomen
- Constipation
- Backache
- Pain in lower limbs
How it's Diagnosis done?
Uterine fibroids are usually diagnosed accidently during a routine or pre-natal pelvic examination.Following diagnostic tests are suggested for the women who present with sign and symptoms of uterine fibroids;
- Ultrasound: an abdominal ultrasound scan is done to confirm uterine fibroids. The number, size and site of uterine fibroid are also confirmed by ultrasound.
- MRI: it is an imaging test used to have a better visual about the uterine fibroids. This test also helps in identifying different types fibroids.
- Hystreosonography: it is a saline infusion sonogram that uses saline to expand the uterine cavity. This facilitates better imaging of uterus. This test is only recommended in symptomatic women in whom conventional ultrasound doesn't reveal any findings
- Hysterosalpingography: in this test, a radio opaque dye is infused in the uterine cavity and X-ray images are taken. This test is usually recommended in women who have fertility issues along with uterine fibroids
- Hystereoscopy: during this test a hystereoscope( small lighted telescope) is inserted into cervix and used to visualized the uterine cavity and fallopian tubes.
- Lab investigations include routine blood tests that are conducted to rule out possibility of other medical conditions responsible for heavy vaginal bleeding, such as anemia.
Can fibroids removed if they are located inside the uterus?
Fibroids that are located inside the uterus are known as sub mucosal fibroids. Hysteroscopic myomectomy and endometrial ablation are the minimal invasive procedures that are used to treat sub mucosal fibroids.
Treatments available for uterine fibroid
- Medications
Treatment of uterine fibroids can be done by prescribing medications. The medications used for treating uterine fibroids targets the reproductive hormones that in turn regulate the menstrual cycle and result in shrinking of fibroids. Gonadotropin releasing hormone agonists such as lupron, synarelare most commonly used. A progestin-releasing intrauterine device is used for women having heavy menstrual bleeding. Pain killers/NSAIDS are prescribed to reduce pain, though they don't shrink the size of the fibroid. - Non-invasive procedures
MRI-guided focused ultrasound surgery is a non-invasive procedure that is done on outpatient basis to treat uterine fibroids. During this procedure a high energy ultrasound transducer is used to target sound waves on the fibroid. This generates heat inside the fibroid and destruct the fibroid. - Minimal Access Surgeries for Uterine Fibroids
